What is the W-9S form?

This form is used to determine a student's correct identification number. This could be a Social Security Number (SSN) or an Individual Taxpayer Identification Number (ITIN). To provide accurate information on your 1098-T Florida State University requires your correct identification number. If you do not have one of the aforementioned identification numbers, do one of the following:

  • To apply for an SSN, visit: http://www.ssa.gov/forms and follow the instructions on the Social Security Card application.
  • To apply for an ITIN, visit: www.irs.gov and search for the W-7 form. Follow the instructions listed on the form.

Only complete and submit a W-9S form if you have been contacted by the University and were instructed to complete one.

How do I complete the W-9S?

Complete the form. This means providing the following:

  • Your name
  • Your address
  • Your Taxpayer ID number
  • Your FSUID
  • Checking the applicable checkbox
  • Your signature and date of signature

Once you have completed the form, please submit it to the Office of Student Finance in person, by mail, or through the Nifty Nole dropbox. Forms can be sent via Nifty Nole (https://nifty.fsu.edu/) addressed to studentfinance@fsu.edu. Forms can be mailed to:
